Teaching NeuroImages: adrenoleukodystrophy presenting as raised intracranial pressure.

چکیده
A 7-year-old boy presented with acute onset headache, vomiting, and visual deterioration 2 weeks previously without any cognitive or behavioral changes. Optic fundi showed papilledema, and there were no pyramidal signs or gait disturbances. The remainder of the neurologic examination was normal. CSF protein was 174 mg/dL. Although MRI features (figure) were consistent with adrenoleukodystrophy (ALD), the atypical presentation prompted consideration of alternate diagnoses and therapies. Subsequent progressive vision and hearing loss, bilateral pyramidal signs, and elevated plasma very-long-chain fatty acids confirmed ALD. Childhood cerebral ALD can present acutely, but rarely with raised intracranial pressure.1 Elevated CSF protein and mass effect due to fulminant demyelination and inflammation is a possible cause. Familiarity with atypical clinical courses2 and classic MRI features may avoid unnecessary investigations and delayed diagnosis.

Teaching neuroimages: leptomeningeal and pachymeningeal enhancement in a patient with spontaneous CSF leak.
A 35-year-old man presented with fever, headache, and vomiting. Lumbar puncture showed low pressure (50 mm H2O), raised levels of protein (4.96 g/L) and white cell count (2,500; 85% polymorphs), and a glucose level of 1.61 mmol/L. MRI demonstrated diffuse leptomeningeal and pachymeningeal enhancement (figure, A), suggesting meningitis and intracranial hypotension due to spontaneous skull base C...
متن کاملTeaching NeuroImages: Acute generalized suppression on continuous EEG heralds clinical and radiologic deterioration.
A 79-year-old woman was admitted to the hospital following 2 generalized tonic clonic seizures and a large right intracerebral hemorrhage (ICH). She deteriorated 72 hours later, losing her right pupillary reflex. A CT scan at that time showed stable ICH volume with effacement of the right suprasellar cistern, concerning for impending herniation. Interestingly, her continuous EEG (cEEG) showed a...

A previously well 33-year-old man with no history of trauma or substance abuse presented with poor right eye visual acuity, somnolence, and vomiting several hours after sudden onset of severe, persistent headache. Examination revealed only a right relative afferent papillary defect and subretinal blood on funduscopy (Terson syndrome, figure 1A). Hunt and Hess grade was 3. CT showed right subdur...
متن کاملManaging children with raised intracranial pressure: part two (brain tumours and intracranial bleeds).
This article is the second of two examining the causes of increased intracranial pressure in children. Key features and management associated with brain tumours and intracranial bleeds are highlighted. The conditions are accompanied by illustrative case studies to give an idea of what children's nurses may encounter in a patient presenting with raised intracranial pressure. Part one, published ...
متن کاملAlveolar soft-part sarcoma presenting with multiple intracranial metastases.
A 28-year-old man presented with history of raised intracranial pressure and one episode of generalized tonic clonic seizures. Computed Tomogram revealed multiple contrast enhancing intracranial lesions. Biopsy of one of the lesions was reported as metastatic alveolar soft part sarcoma. He was advised whole brain radiotherapy.
